This position is located in the Palliative Care Department with the Palliative Care Team

What You Will Do

As an Associate with AMITA Health, you will have the opportunity to
Coordinates home care and/or hospice intake activities and resources.
Responsibilities:
  • Completes referral process.
  • Explains home health and/or hospice services and facilitates continuity of care.
  • Assists with communication or payer sources. Determines benefit eligibility.
  • Obtains, records and disseminates referral information.
  • Documents clinical records to ensure compliance with regulatory standards, agency policies and procedures.

What You Will Need

Licenses/Certifications/Registration:
  • Required Credential(s):
    • Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date.


Education:
  • Diploma
    • Diploma from an accredited school/college of nursing required

Preferred Experience:
  • Hospice and Palliative Care
