What You Will Do

As an Associate with AMITA Health, you will have the opportunity to
Coordinates the overall interdisciplinary plan of care for patient, from admission to discharge.
You will:
  • Creates plan for care across the continuum, integrating patient/family preferences and values.
  • Monitors patient care through assessments, evaluations and/or patient records.
  • Advocates for resources and removal of barriers.
  • Maintains ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.

What You Will Need

Licenses/Certifications/Registration:
  • Required Credential(s):
      • Licensed Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date.

Education:
  • Diploma
    • Diploma from an accredited school/college of nursing required

Work Experience:
  • Three years of experience in case management and/or clinical nursing preferred.
  • RN position - • RN with current licensure in the State of Illinois, Bachelors' degree preferred, Certification in Case Management preferred.
    • Minimum of 5 years recent applicable clinical experience in acute hospital setting with the clinical expertise to act as a resource to assigned areas. Prior experience in Case Management, Utilization Management or Discharge Planning strongly preferred.
    • Excellent verbal communication and interpersonal skills needed to work in a collaborative practice setting with an interdisciplinary team and to interact effectively with patients, physicians, payers and other external agencies.
    • The analytical skills necessary to understand the relevance and impact of data and clinical information to support the appropriate utilization of resources associated with cost and quality of patient care.
    • Excellent skills in the following areas: critical thinking, problem identification and resolution.
    • Excellent negotiation skills with physicians and others to ensure patients receive appropriate and timely care.
    • Able to perform multiple tasks in an organized manner with a high degree of flexibility required. Ability to make decisions in a fast-paced environment.
    • Self-directed learner; enthusiastically seeks ways to improve patient care and motivate others to participate in improvement activities.
    • Knowledge of and ability to utilize in-house and external resources.
    • Intermediate to advanced computer skills necessary to utilize multiple computer applications such as, Cerner, Meditech, Athena, NextGen, Internet, and Microsoft Office products.
  • Monday-Friday, 8:00AM-4:30PM
