RN-Patient Navigator - Cancer Center 001 - Full-Time,Day - AMITA Cancer 1 - Hinsdale,IL

Mon to Fri 7 am to 4:30 pm. Ability to work events that may take place in evening or on weekends (about 5-8 times per year)

What You Will Do

As an Associate with AMITA Health, you will have the opportunity to
Serves as a patient navigator throughout the continuum of care.
Responsibilities:
  • Functions as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.
  • Coordinates efforts for treatment and promotes quality improvement and educational efforts.
  • Compiles data, tracks outcomes, and makes recommendations for process improvement.
  • Provides referrals/support for follow-up care for patients who are uninsured or underinsured.

What You Will Need

Licenses/Certifications/Registration:
  • Required Credential(s):
    • Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date.
  • Preferred Credential(s):
    • BLS Provider credentialed from the American Heart Association (AHA).

Education:
  • Diploma
    • Diploma from an accredited school/college of nursing required
    • BSN is strongly preferred.

Work Experience:
  • Minimum of 3-5 years demonstrated experience of working in acute care oncology or physician office setting preferred.
  • Infusion experience is strongly preferred.
  • OCN certification highly desired.
  • Certification as a Nurse Navigator or other specialty oncology disease areas, a plus!
