What You Will Do
Job Summary:
Assures the appropriate selection, dosing, administering, and dispensing of medication orders.
Responsibilities:
- Reviews and assesses medication orders to identify, prevent and resolve potential and actual drug related problems. Recommends formulary alternatives when appropriate.
- Compounds and dispenses medications for accurate delivery to patients.
- Provides drug information and education to medical staff and patients.
- Communicates with physicians and other clinicians as necessary to facilitate care.
Qualifications
Licenses/Certifications/Registration:
- Required Credential(s):
- Pharmacist credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date. In good standing.
Education:
- Bachelor's Level Degree
- Bachelor's degree from an AACP accredited college required PharmD preferred
Work Experience:
- One or more years of experience in a hospital setting preferred.
- Bachelors of Science in Pharmacy or PharmD
PGY 1 Residency and/or 3 years equivalent pharmacy experience - This position will rotate between the 1st and 2nd shift primarily. There'll be some 3rd shift and weekend staffing requirements as well. Scheduling is dictated by the needs of the department.
