SUMMARY

Provides accurate and timely dispensing of medications, in all forms, to meet the needs of the individual patient according to professional standards and practices. Provides drug information, as requested, to patients, staff, and physicians. Assists in coordinating and supervising the other pharmacy staff (technicians, inventory personnel, etc).

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Order Processing
  • Accurately transcribes, processes, and fills all medication orders in a timely manner.
  • Compounds prescription orders accurately and efficiently.
  • Clarifies orders with prescriber as necessary.
  • Consults with nursing staff and others regarding patient specific information.
  • Plans and organizes work effectively.
  • Patient Specific Dosing - Pharmacokinetics
  • Demonstrates basic pharmacokinetic knowledge base necessary to monitor patient medication orders for appropriate dosing based on patients age, weight, gender, etc.
  • Checks all medication orders for appropriateness in respect to age, weight, gender, etc.
  • Makes dosage recommendations, subsequent, and adjustments for renally cleared drugs. Calls physician with recommendation and documents per protocol/guidelines.
  • Makes basic clinical decisions accurately and efficiently.
  • Maintains professional and departmental competencies.
  • Answers drug information questions.
  • Dispensing
  • Checks all medication orders precisely and efficiently, which includes label correctness, auxiliary labels, and patient specific labels. If an error is noted, corrects technician and educates them on error. If a system error occurs, brings to the attention of a supervisor immediately.
  • Maintains/completes required dispensing records.
  • Informs appropriate staff if drug shortages occur.
  • Checks all items for placement in automated dispensing cabinets/floor stock area.
  • Checks all crash carts/drawers in a timely manner to assure sufficient supply available at all times.
  • Narcotic Control
  • Fills all narcotics orders accurately and per policy and procedure.
  • Maintains accurate and complete controlled substance records.
  • Disposes of narcotics per regulations and policies.
  • Quality Assurance
  • Identifies areas where there may be deficiencies that are affecting the delivery of quality pharmaceutical care and brings them to supervisors attention.
  • Reports and/or completes form for adverse drug reactions.
  • Reports medication errors to supervisor.
  • Participates in audits or DUE studies as assigned.
  • Supervises Technicians
  • Checks all technicians filling in accordance with hospital policy.
  • Reassigns technician duties as needed based on workload.
  • Takes the initiative to contact pharmacists/technicians if necessary to cover for sick calls or emergencies.
  • Makes sure drug orders have been placed as needed or out of stock/non-formulary medications obtained per department guidelines along with appropriate documentation. Informs ordering person when office supplies are running low. Knows how to order drugs from wholesaler when necessary per department procedures.

    Education and/or Experience
    • 1-3 years experience with Bachelor's degree PharmD preferred


    Computer Skills
    • Proficient with Windows-style applications and keyboard.
    • Computer skills and dexterity required for data entry and retrieval of information.

    • State of Illinois Registered Pharmacist License
