SUMMARY

The Speech Language Pathologist II identifies the speech, language or motor/swallowing disorder of the assigned patients, and assures this data is communicated and reported. Completes patient initial assessment and other appropriate evaluations accurately and thoroughly while being audited by random observation or performance improvement monitoring. Evaluates and treats patients using the principles and practices of speech-language pathology and in accordance with the patient's age, developmental level and diagnosis. Collaborates with the interdisciplinary team. Patient and family education is an integral part of the service offered.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Evaluates patients and determines causal factors of speech and language impairments for pediatric, adolescent, adult and geriatric populations as prescribed by a licensed physician.
  • Gathers data related to patient's communication or swallowing disorder and selects appropriate evaluation tools for the purpose of assessment. Administers diagnostic testing procedures to determine the extent of the client's impairment and the prognosis for improvement.
  • Develops and implements individualized treatment programs for patients diagnosed as needing speech, language, or swallowing intervention by utilizing resources from other members of the health care team. Develops and maintains long-term and short-term treatment objectives for each patient. Re-evaluates and modifies treatment programs as warranted.
  • Educates patients' family members/significant others to ensure understanding of speech, language and swallowing impairment(s) and treatment.
  • Recommends modification or changes in the patient's treatment program to the referring physician based on evaluation of progress. Confers as necessary with physicians concerning alternatives available. Recommends to the referring physician the referral of patients to specialists in medicine, dentistry, psychology or other related areas.
  • Prepares written/computerized documentation as required by the profession and the department, such as assessment, evaluation results, individualized treatment plans, treatment session analysis, progress reports, discharge summaries, etc.
  • Communicates observations, facts and comments with nursing and interdisciplinary team members, individually, and through interdisciplinary team meetings to ensure efficient coordination of the overall health care plan.
  • Makes efficient use of direct and non-direct patient care time. Adjusts schedule to meet patient needs. Picks up additional patients and assists co-workers as needed. Takes initiative to improve department productivity.
  • Assists with care and maintenance of department equipment and supplies. Maintains appropriate stock levels of supplies or communicates with appropirate personnel.
  • Provides training, mentorship, and quality review to Speech and Language Pathologist 1.
  • Provides day-to-day training/supervision of graduate or CFY candidates as assigned.

    QUALIFICATIONS

    Education and/or Experience

    Master's degree in Speech Language Pathology from an accredited institution required. Three years of experience required.

    Computer Skills

    Proficient in Microsoft Office software.

    Certificates, Licenses, Registrations

    Certification of Clinical Competence from ASHA, or eligibility for certification required. If not certified upon hire, must meet eligibility requirements for the prerequisite Clinical Fellowship Year "CFY" within the nine month period allowed. Current Illinois license or eligibility for licensure required; if not licensed upon hire, must meet the requirements for the prerequisite "CFY" within the nine month time frame allowed. CPR Certification required
