What You Will Do

As an Associate with AMITA Health, you will have the opportunity to

Coordinates comprehensive plan of care, counseling services and case management for patients.

Responsibilities:
  • Oversees primary needs of patient, including assessment of client and family/caregivers, and develops comprehensive plan of care.
  • Provides assigned patient counseling services and case management.
  • Provides primary home care/consultation to complex client and family needs within area of expertise and in collaboration with physician.
  • Monitors patient care through assessments, evaluations and/or patient records. Advocates for resources and removal of barriers.
  • Maintains ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.
  • Willingness to travel in neighborhoods on the north, west and south side of Chicago to engage in home visits with clients as part of mandatory position requirements.

What You Will Need

Licenses/Certifications/Registration:
  • Preferred Credential(s):
    • Counselor credentialed from the Illinois Department of Financial and Professional Regulation.
    • Social Worker credentialed from the Illinois Department of Financial and Professional Regulation.
    • Case Manager credentialed from the Commission for Case Manager Certification (CCMC).

Education:
  • High school diploma/GED with 2 years of experience, or Associate's degree, or Technical degree required
  • Bachelors degree in Psychology, Social Work or related field highly desired.

Work Experience:
  • Bachelors degree minimum with experience working in human services.
  • Masters in social work or psychology preferable.
  • CADC highly desirable.
  • History of housing case management and knowledge of chronic illnesses is highly desirable.
  • Experience working with EMR systems preferred.
  • Rule 132 experience preferred
  • Valid IL Driver's License
