Job Description

Job Summary:

Coordinates comprehensive plan of care for patients in psychiatric program.

Responsibilities:
  • Oversees primary needs of patient, including assessment of client and family/caregivers, and develops comprehensive plan of care. Adheres to agency and regulatory standards in oversight of psychiatric program.
  • Provides primary home care/consultation to complex client/family needs within area of expertise and in collaboration with physician.
  • Monitors patient care through assessments, evaluations and/or patient records. Advocates for resources and removal of barriers.
  • Maintains ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.

Qualifications


Education:
  • Bachelor's Level Degree
    • Bachelor's degree or meets the qualifications as a RSA (Rehabilitative Services Associate) under the DHS standards required

Work Experience:
  • One year of relevant experience working in community mental health or in the behavioral health field preferred.
