What You Will Do
Job Summary:
Coordinates comprehensive plan of care for patients in psychiatric program.
Responsibilities:
- Oversees primary needs of patient, including assessment of client and family/caregivers, and develops comprehensive plan of care. Adheres to agency and regulatory standards in oversight of psychiatric program.
- Provides primary home care/consultation to complex client/family needs within area of expertise and in collaboration with physician.
- Monitors patient care through assessments, evaluations and/or patient records. Advocates for resources and removal of barriers.
- Maintains ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.
Qualifications
Education:
- Bachelor's Level Degree, Bachelor's degree or meets the qualifications as a RSA (Rehabilitative Services Associate) under the DHS standards required
Work Experience:
- One year of relevant experience working in community mental health or in the behavioral health field preferred.
- Experience working with/facilitating group therapy sessions highly preferred
