SUMMARY

Under general supervision, coordinates all facets of the educational program for adolescents including education, assessments, curriculum development; liaison with the home school, school staffings, communication with other members of the multidisciplinary treatment team and parents, record maintenance and billing.



ESSENTIAL DUTIES AND RESPONSIBILITIES

A. Provides direct educational services to adolescents in conjunction with their home school academic programs. Develops educational curriculum or programs for adolescents not currently enrolled in a school.

1. Serves as a liaison to coordinate educational services with the patient's home schools. Contacts personnel at the home to obtain educational background including academic, social and behavioral information.

2. Assesses the educational needs of the patient, analyzes this information, and develops the educational assessment, which is part of the medical record.

3. Completes/forwards statement for Educational Services form in a timely manner.

4. Coordinates and participates in school staffings which includes obtaining and copying all necessary reports to school personnel, obtaining the physician's recommendations for educational services, scheduling necessary personnel to attend and obtaining the discharge plan. This staffing may take place at PSJMC or at the home school. Completes required to attend school staffing form and submits to manager.

5. Participates in weekly Team Conferences with the psychiatrist and other members of the multidisciplinary team to develop treatment goals for the patients. Attends other staffings on the Adolescent Unit.

6 . Participates in all departmental activities as needed. These activities include, but are not limited to, departmental meetings, quality improvement activities, policy and procedure development, implementation and evaluation, the budgetary process,

Education and/or Experience

Bachelor's degree in education is required. One year of classroom experience, experience in a residential treatment center or institutional setting is preferred.

Must have a Type 9 or Type 10 Certificate.
