Provide direct observation one-on-one with patients as required/requested by Directors/Managers. Direct observation is provided through the use of Tele-Health Equipment placed in the room of the patient(s) being observed. Inform unit staff whenever additional assistance is required in the individual patient room.



Duties & Responsibilities:

  • Promote client safety and relay basic information to nursing staff


  • Use the Pro-net Call Answer Terminal (CAT) system to communicate urgent needs to staff ie: assist patient to bathroom, patient attempting to exit bed.


  • Observe the patient to ensure a safe environment.


  • Participate in educational programs and integrates learning into job role.


  • Communicate pertinent patient information to RN and other staff including changes in patient's condition and needs, and report patient care and/or unit issues up the chain of command to insure prompt attention and resolution. Partner with other members of the care team to achieve patient plan of care/goals while maintaining patient privacy.


  • Document patient behavior, interventions, and patient response on the observation log per policy and maintain confidentiality of patient information.


Qualifications:

  • High school diploma or G.E.D. required .


  • 6 months of health care experience required.


  • Basic computer knowledge required .


  • Certified Nursing Assistant with a current Illinois License required. Basic Life Support (BLS) certification required.
