SUMMARY
The Director, Social Services provides direction and supervision of the facility's program for Social Services including regulatory compliance, supervision of staff and coordination of services with other departments.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Directs and oversees the psych-social programs at the facility.
- Manages the completion and update of each resident's social history, social assessment and discharge plan summary according to existing regulations.
- Assists residents and families by providing information concerning available services both within the facility and in the community.
- Assures that services and programs meet all applicable regulations.
- Provides/directs social service, counseling intervention and clinical documentation as required by regulations to meet resident needs.
- Meets material needs of residents, i.e. clothing, etc.
- Participates in orientation of staff, reviewing Resident Rights.
- Responsible for hiring, orientation, evaluation, supervision, scheduling, counseling and disciplining department staff.
- Provides for quality assurance within the department.
- Develops job descriptions for department as necessary.
- Serves as a member of QAA committee and attends meetings and care plan conferences as part of the interdisciplinary team.
- Responsible for completing sections on the MDS for social services RAPS and Psych-Social to establish social service triggers as required by regulation.
- Other duties may be assigned.
QUALIFICATIONS
Education and/or Experience
- Bachelor's degree in Social Services or related field.
- Minimum one year of experience in long-term care or Social Services.
Computer Skills
- Proficient with Microsoft Office software (Word, Excel, Access, PowerPoint, Outlook, etc.).
