SUMMARY

The Wound Ostomy Nurse is a Registered Professional Nurse (RN) that participates within the scope of the Illinois Nurse Practice Act to ensure the delivery of comprehensive, high quality, individualized nursing care, based upon knowledge and experience, the nursing process and leadership skills. The Wound Ostomy Nurse incorporates the carative factors of human care and performs established procedures in accordance with the organization's and the nursing division's policies, procedures, and standards of care.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Provides consultative visits to referred patients, to assess skin, wound, and/or ostomy issues. Recommends and initiates pressure ulcer prevention interventions and appropriate treatment plans, and documents. Consults with physicians, nurses, and interdisciplinary team. Re-evaluates patients and provides direct patient care.


  • Integrates evidence-based knowledge into the care of patients with wound, ostomy, and continence issues; establishes and maintains standards of practice according to the Wound, Ostomy, and Continence Nursing (WOCN) and Presence Health guidelines.


  • Provides formal and informal education and is a resource related to the management of skin, wound, and ostomy issues.


  • Participates in the orientation of nursing personnel.


  • Implements established policies, procedures, and standards of care.


  • QUALIFICATIONS

    Education and/or Experience

    Graduate of an accredited nursing program. Three years of Medical-Surgical nursing experience with demonstrated clinical/technical ability, teaching, and communication skills.

    Bachelors of Science in Nursing preferred

    Computer Skills

    Knowledge of Microsoft Office software and Electronic Medical systems required.

    Experience of EPIC preferred.

    Certificates, Licenses, Registrations

    Registered Nurse with current Illinois License. Basic Life Saving (BLS) certification is required. Certification from an accredited wound or ostomy education program required within one year of employment.
