SUMMARY

Under the general supervision of the Director of Social Services, the Director of Memory Care is responsible for the unit compliance with the Alzheimer Unit Regulations, policy design and implementation for the unit, staff implementation of the regulations, appropriateness of admissions and residents who are care for in the unit, and Alzheimer and Dementia training of all staff in the facility. Performs management functions relating to but not limited to: planning, budgeting, policy, staffing and community relations in conjunction with the Director of Social Services, the Director of Activities and the Director of Nursing.



ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Develops policy and procedures that comply with all state and federal regulations for the Unit.
  • Assists nursing and activities to prepare Unit staffing and budget, manages both nursing and activity staf as their duties apply to Alzheimer's/dementia regulations and facility HR policy.
  • Coordinates with the activity and nursing directors for all hiring, training, and evaluation of Unit staff.
  • Recommends changes in policy, procedures, programs, and organization to Administration.
  • Conducts Memory Care Unit staff meetings.
  • Contacts, develops and maintains community referral sources.
  • Coordinates the family support group meetings and outside support groups for Alzheimer and dementia.
  • Maintains communication with resident' families.
  • Establishes and maintains active relationships with related professional groups, in continuing education on Alzheimer's and dementia for professional growth and self-development.
  • Responsible for knowing and doing what is right and reporting any legal, ethical, and safety concerns to the appropriate party.
  • Responsible for participation in the RAI process including but not limited to completion of assigned sections of the MDS, Care Area Assessments (CAA) and care planning.
  • Monitors the overall supervision of residents.
  • Be familiar with emergency procedures of the institution and be able to carry out necessary procedures for resident safety.
  • Knows and understands Resident Rights.
  • Accepts and promotes the philosophy of Presence Health in the care of the elderly.
  • Coordinates patient and resident activities that match the individual's interests, cognitive ability, memory, attention span, language, reasoning ability, and physical function.
  • Monitors staff performance regarding personalized approaches to address behavioral expressions of unmet needs.


SUPERVISORY RESPONSIBILITIES

Directly supervises employee(s). Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

QUALIFICATIONS

Education and/or Experience
  • Bachelor's degree in Social Services or related field.
  • Minimum one year of experience in long-term care or Social Services.
  • Specific training or education in the provision of dementia care and services.


Computer Skills
  • Proficient with Microsoft Office software (Word, Excel, Access, PowerPoint, Outlook).
