SUMMARY

Provide regional coordination of the system-designed stroke programs at the ministry level in accordance with TJC Stroke Center for Excellence. Coordinate and assure the implementation of system-wide acute stroke assessment and treatment response with local Stroke Team members. Develop and maintain clinical protocols, and facilitate the development of clinical practice guidelines for stroke care.



ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Serve as a clinical resource to hospital staff personnel, other health care professionals within the organization and community for complex or highly specialized patient care cases.
  • Ensure compliance with the Joint Commission Advanced Disease-Specific Care Certification standards to maintain certification and completion of all intracycle reports, coordinating submission to the accrediting body within system leadership within required time frame.
  • Monitor adherence to protocols, tracks variances from protocols and reports outcome data to Local Stroke Team, appropriate System teams, and various internal and external parties, including the Joint Commission in accordance with the established Performance Improvement Plan.
  • Coordinate and assure implementation of system wide acute stroke assessment and treatment response with the Local Stroke Team members.
  • In collaboration with System Stroke Collaborative, develop and maintain clinical protocols; updates as necessary to reflect current evidence-based practice.
  • Coordinate neurology stroke patient care with special attention to rehab and discharge planning needs. Collects and tracks data for performance improvement, education, and certification
  • Participate in the identification of learning needs; coordination of planning, and implementation; and compliance with monitoring of educational programs for Stroke Team members, the Emergency Department staff/physicians, Emergency Medical Services personnel and those areas involved in stroke care and employees/physicians throughout the hospital.
  • Facilitate the planning and implementation of community outreach programs for stroke awareness and prevention promotion, coordinating efforts across the system.
  • Serve as a clinical resource to hospital staff personnel, other health care professionals within the organization and community for complex or highly specialized patient care cases.

    Good organizational and leadership skills.
    Excellent human relations and oral/written communication ability and problem-solving skills required.

    Education and/or Experience
    Bachelor's Degree in Nursing or Health related field required. Master's degree preferred.

    Three years of clinical experience with neurovascular patients and stroke program certification required.

    Education and training experience preferred.

    Clinical Nurse Specialist/Nurse Practitioner preferred.

    Computer Skills
    Solid computer skills required. Proficient in Microsoft Word, Excel, PowerPoint and Access.

    Certificates, Licenses, Registrations
    Current RN license required.
    CRRN, CCRN or CNRN preferred
