SUMMARY

The Central Intake Registered Nurse receives and processes initial patient referral information for assigned Presence Health facilities. Coordinates with physician offices, emergency department, other healthcare facilities, and/or other ancillary areas. Utilizes the nursing process (assess, plan, implement and evaluate) and management process (plan, organize, direct and control) to provide a framework for decision-making.



ESSENTIAL DUTIES AND RESPONSIBILITIES

Processes initial orders for referred patients to the facility; reviews intake information for completeness; inputs data into computer system; and contacts potential clients' physicians for patient information as needed.

Serves as a resource to nurse liaisons, physicians and discharge planners regarding the facility, private insurance and public aid benefits. Communicates agency services and philosophy to all referral sources and all callers seeking care information.

Ensures maximum third party reimbursement through direct oversight of insurance verification and authorization process.

Facilitates nurse-to-nurse reports if required, adding disciplines and physician orders when indicated.

Ensures that the highest quality service/customer support is provided to Presence Health clients.

Promotes effective public relations for Presence Health by initiating and maintaining ongoing relationships with discharge planners, physicians and community resources.

Assesses each patient's biophysical, psychological, environmental, self-care, educational and discharge planning needs based on age and developmental status.

Education and/or Experience
Graduate of an accredited nursing program required. Three years of clinical experience required.

Computer Skills

Certificates, Licenses, Registrations
Registered Nurse with current Illinois License required. Basic Life Support (BLS) certification required.
