SUMMARY
The Social Worker I shares the responsibility of identifying social and emotional needs of each client. The position assists with arranging services to meet the needs of residents, providing liaison between the client, family, facility staff, and the community.

QUALIFICATIONS

Education and/or Experience
Bachelor's degree required, Master's degree strongly preferred (Master's may be required based on license). Preferred one year of hospital social work, relevant field work placement in the mental health field or home care experience is preferred.
Computer Skills
Certificates, Licenses, Registrations
Current State of Illinois license as a Licensed Social Worker (LSW) required. Current Cardiopulmonary Resuscitation Certification per department policy. Membership in National Association of Social Workers is preferred.
Proficient in Microsoft Office software.
