What You Will Do
As an Associate with AMITA Health, you will have the opportunity to
Evaluates and conducts medically prescribed physical therapy treatment programs.
Responsibilities:
- Performs initial and on-going assessments of patient's condition.
- Establishes, revises and evaluates a plan of care that is appropriate to problems identified and involves the patient/family.
- Performs therapy interventions utilizing standard physical therapy techniques and skills as appropriate for the condition of the patient.
- Oversees activities of physical therapy assistants, students and other support personnel.
What You Will Need
Licenses/Certifications/Registration:
- Required Credential(s):
- BLS Provider credentialed from the American Heart Association (AHA) obtained prior to hire date or job transfer date.
- Drivers License credentialed from the Illinois Department of Motor Vehicles (IL-DMV) obtained prior to hire date or job transfer date.
- Rehab Therapist specializing in Physical Therapy (RPT) credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
Education:
- Diploma
- Graduate of a physical therapy school approved by the American Physical Therapy Association or the equivalent required
Work Experience:
- One year of physical therapy experience in a setting that involves direct patient care required..
- Licensed Physical Therapist with at least 2 years of experience, preferably home care experience.
- Weekends Days Saturday and Sunday resource on-call position
