What You Will Do

As an Associate with AMITA Health, you will have the opportunity to
Evaluates and conducts medically prescribed physical therapy treatment programs.
Responsibilities:

• Performs initial and on-going assessments of patient's condition.
• Establishes, revises and evaluates a plan of care that is appropriate to problems identified and involves the patient/family.
• Performs therapy interventions utilizing standard physical therapy techniques and skills as appropriate for the condition of the patient.
• Oversees activities of physical therapy assistants, students and other support personnel.

What You Will Need

Licenses/Certifications/Registration:

• Required Credential(s):
  • BLS Provider credentialed from the American Heart Association (AHA) obtained prior to hire date or job transfer date.
  • Drivers License credentialed from the Illinois Department of Motor Vehicles (IL-DMV) obtained prior to hire date or job transfer date.
  • Rehab Therapist specializing in Physical Therapy (RPT) credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.

Education:

• Diploma
  • Graduate of a physical therapy school approved by the American Physical Therapy Association or the equivalent required

Work Experience:

• One year of physical therapy experience in a setting that involves direct patient care required..
• Licensed Physical Therapist with at least 2 years of experience, preferably home care experience.
• Weekends Days Saturday and Sunday resource on-call position
