SUMMARY

The Social Worker II shares the responsibility of identifying social and emotional needs of each client. The position assists with arranging services to meet the needs of residents, providing liaison between the client, family, facility staff, and the community. Assess all patients admitted to the Mental Health department, gathers information from available sources including family, agencies, friends and others, analyzes the information, formulates treatment recommendations, communicates pertinent information to other members of the interdisciplinary treatment team, participates in the development of the patient's treatment plan, coordinates or provides services including individual, group, family, marital and/or group psychotherapy and discharge planning. Serves as a resource to staff and community regarding psychosocial issues. Participates in Hospital and departmental planning committees and teams.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Evaluates and assesses the social, cognitive, emotional, behavioral, and environmental status of residents as prescribed by regulatory agencies.

  • Communicates with patient, family, relevant professionals, programs, agencies and others to help client secure service needs, make preferences known, and determine appropriate placement within the facility and in the community.

  • Provides ongoing counseling and emotional/social support in relation to health/social needs, and plan of care, to enable them to cope with difficulties and properly utilize services.

  • Documents interventions and updates care plans as prescribed by regulatory agencies, and as problem situations occur.

  • Participates in ongoing evaluation of residents' needs and goals through multi-disciplinary care planning.

  • Coordinates patient billing process for social service treatment, discharge planning and maintain good public relations with community services.

  • Provides assistance with and acts a resource to residents and families concerning public aid and legal documents, including but not limited to living wills, Do Not Resuscitate (DNRs), and power of attorney for health care.

  • Consults with and keep director informed of activities, needs, and problems.

  • Assists in care and maintenance of department equipment and supplies.

  • Serves as a resource to Medical Center staff and the community by providing in-service education programs and psychosocial information as requested. Attends meetings and participate on committees and task forces as directed.

  • Orients and supervises field placement students in the completion of their practicum.

  • Assists in verifying insurance coverage for patients and provides clinical updates to insurance companies.

    QUALIFICATIONS

    Education and/or Experience

    Master's degree in Social Work from a school approved by the Council on Social Work Education or related field.

    Minimum two years of hospital social work, relevant field work placement in the mental health field or home care experience is preferred. Alternatively, Psychiatric Registered Nurse with 5 years of experience.

    Computer Skills

    Proficient in Microsoft Office software (Word and Excel).

    Certificates, Licenses, Registrations

    Current State of Illinois license as a Licensed Social Worker (LSW), LCPC, LPC, LMFT, or as a Licensed Clinical Social Worker (LCSW) required. Current Cardiopulmonary Resuscitation Certification per department policy. Valid Driver's License.

    Crisis Prevention Institute Certification required within 90 days of hire.
