Job Description
Job Summary:
Provides social services to patients, families and the community as requested or deemed appropriate.
Responsibilities:
- Understands and implements social work principles and code of ethics.
- Conducts discharge planning, crisis counseling, community resource information and education.
- Works effectively within a multidisciplinary team environment.
Qualifications
Education:
- Bachelor's Level Degree
- Bachelor's degree in a behavioral science or related field required
Work Experience:
- One year of experience working in community mental health or in the behavioral health field preferred. Some crisis experience a plus.
- Work is done remotely via telephone from home, no office-based work but must be able to attend one monthly team meeting.
