Job Description

Job Summary:

Provides social services to patients, families and the community as requested or deemed appropriate.

Responsibilities:
  • Understands and implements social work principles and code of ethics.
  • Conducts discharge planning, crisis counseling, community resource information and education.
  • Works effectively within a multidisciplinary team environment.

Qualifications


Education:
  • Bachelor's Level Degree
    • Bachelor's degree in a behavioral science or related field required

Work Experience:
  • One year of experience working in community mental health or in the behavioral health field preferred. Some crisis experience a plus.
  • Work is done remotely via telephone from home, no office-based work but must be able to attend one monthly team meeting.
