Job Description

Job Summary:

Provides coordination of patient care throughout the healthcare continuum.

Responsibilities:

• Provides coordination of communications with patients, families, physicians, and healthcare team members.
• Coordinates patient scheduling with appropriate resources or clinical services and maintains required documentation.
• Supports standard patient access initiatives across the healthcare continuum.
• Provides support with navigating the healthcare system and assists patients with utilizing available community support services.

Qualifications

Education:

• Diploma
  • Graduate of an accredited school of nursing required
  • or
  • Master's Degree in Social Work

Work Experience:

• Three years of experience in case management and/or clinical experience preferred.
