SUMMARY

The Resource & Referral Line Clinician shares the responsibility of identifying social and emotional needs of each client. The position assists with arranging services to meet the needs of residents, providing liaison between the client, family, facility staff, and the community.

The primary responsibility of this position is to answer the Resource & referral line, provide appropriate resources & referrals to the caller and arrange for a warm handoff when possible. This role will include providing high level community case management and provide follow up with clients when needed. The Resource & Referral line clinician shares the responsibility of identifying resource needs of each client. This position serves as a resource to staff and community regarding psychosocial issues as well as serves as the primary point person for any transfer requests to facilitate patient transfers from other facilities. The Resource & Referral Line Clinician gathers information from available sources including family, agencies, collaterals and others, analyzes the information, reviews information with psychiatrist regarding disposition and communicates pertinent information to other members of the interdisciplinary team when reviewing transfer requests. This position participates in Hospital and departmental planning committees and teams.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Answers Community Resource & Referral line and provides appropriate resources to callers. Facilitates linkage for callers to the appropriate resource via a warm hand off to the referred agency or service.
  • Provides case management and emotional/social support in relation to health/social needs, to enable them to successfully access services. Case management duties to include follow up calls to callers to ensure linkage to appropriate resource has occurred.
  • Maintains Resource & Referral data base of number of referrals to each referral source, and provides feedback and updates to the Kane County Behavioral Health Council.
  • Responsible for data collection and analysis for Community Resource & Referral line and for transfer requests in BHS Intake.
  • Reviews and assesses the social, cognitive, emotional, behavioral, and environmental status of patients as provided by facilities seeking patient transfer, as prescribed by regulatory agencies . Documents interventions and updates for patient transfers and assessments, as prescribed by regulatory agencies, and as problem situations occur.

    QUALIFICATIONS

    Education and/or Experience

    Master's Degree in Social Work or Counseling is required.

    State Licensure in Social Work (LSW) or Licensed Professional Counselor (LPC) is required, LCSW or LCPC is preferred.

    Work history of 1-3 years in behavioral health is required; 3-5 years preferred.

    Previous supervisory experience preferred.

    Computer Skills

    Typing, word processing and general keyboard skill required. Spreadsheet and presentation building skills preferred.
