SUMMARY

The Dietitian provides optimal nutritional care for patients. Develops, implements, monitors and evaluates nutritional treatment plans in collaboration with the multidisciplinary health care team. Participates as a health care team member and supervises the activities of support personnel.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Assesses complex patients' nutritional status independently and appropriately following the American with Disabilities Acts' evidenced based practice guidelines. Develops goals and an intervention plan designed to address progress of patient towards goals and ensure desired outcomes are being met.

  • Determines outcome indicators that are relevant to the patient's needs, nutrition diagnosis, nutrition goals and disease state and condition. Monitors patient progress, measuring and evaluating progress on a planned schedule until discharge. Identifies changes in outcome indicators such as weight, feeding ability and changes in clinical and functional status and redefines outcomes as part of patient monitoring.

  • Collaborates with physicians and other members of the health care team to provide quality patient care. Accurately identifies and notifies team and physician of necessary modifications to food, nutrient intake and route of administration. Acts as an informational/educational resource to nursing staff, medical personnel and patients.

  • Communicates and documents patients' nutritional care plan, nutritional progress, and diet instruction information in the medical record, according to department standards.

  • Serves as clinical nutrition expert and assumes responsibility for assigned patient care floor or specialty area. Attends scheduled floor meetings and participates in health care team rounds/conferences. Instructs Dietitians Assistants and Diet Technicians in patient assignments, charting, diet consultations, and calorie counts. Participates in community outreach initiatives as assigned.

  • Documents nutritional services provided and patient outcomes and helps support with documentation/other for the departmental hospital Quality Assurance and Performance Improvement (QAPI) program.


  • Adheres to downtime procedures and to established departmental policies, procedures, and objectives.


  • Performs both essential and non-essential job functions in a safe manner.


  • Enhances professional growth and development by accessing educational programs, job related literature, in-service meetings, and workshops/seminars.

    QUALIFICATIONS

    Education and/or Experience

    Bachelor's degree in Food and Nutrition, Clinical Nutrition or equivalent required. Ability to speak and write English to the extent necessary for safe and efficient performance of the job.

    One to three years of work related experience preferred.

    Computer Skills

    General office computer skills required.
