This position is for Critical Care. Nights and weekends. Schedule may vary.

SUMMARY

The Nurse Practitioner (NP) provides patient care services and acts as a program leader and change agent in ensuring the delivery of quality patient care. Assesses patient care and the care delivery system, and develops and revises the plan of care, in collaboration with the physician, patient and family, other health care team members and outside agencies. Plans and conducts educational programs, revises program clinical guidelines/protocols and policies, and performs professional and technical nursing functions.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Conducts focused and/or comprehensive assessments by gathering patient history and identifying normal and abnormal findings on history, physical exam, radiology and lab findings.

  • Coordinates the plan of care with staff, physicians and other healthcare team members to achieve optimal patient outcomes and to facilitate discharge planning for this patient population. Consults with department personnel and leadership and provides clinical expertise in solving complex patient or system problems and to facilitate communication and consensus.


  • Manages a specific caseload of patients by developing and implementing patient management plans and by directing and coordinating care rendered by nursing and other disciplines.

  • Completes daily rounds and pre-procedure evaluations. Monitors symptoms and initiates symptom management interventions in collaboration with the attending physician.

  • Implements individualized treatment plans, evaluates outcomes, and modifies treatment plans (in collaboration with the Primary Care physician, Specialty Physician, and/or Medical Director as indicated),

Education and/or Experience

Master's degree from an accredited Nurse Practitioner Program required.

Computer Skills

Certificates, Licenses, Registrations

Current Illinois Registered Nurse License. Basic Life Support (BLS) or Advanced Cardiovascular Life Support (ACLS) Certification Required. Additional certification in area of specialty may also be required.
