The Mentoring Case Manager is responsible for providing intensive mentoring case management to residents on his/her assigned caseload. The Case Manager will be responsible for coordinating and overseeing participation in existing program offerings, industry-specific training and referrals to Safer’s social and human services support networks. He/she is also part of team that provides group services and interventions.

 

JOB DESCRIPTION:

The Mentoring Case Manager is responsible for:

  • Providing intensive mentoring case management, including coordination among all staff servicing each of the case manager’s assigned residents. This coordination involves but is not limited to security, job developers, basic skills, staff delivery group programs, training and employment
  • Accurately completing all paperwork, procedures and periodic reports required by IDOC, Department of Justice (DOJ) and/or Safer in a timely manner.
  • Managing a caseload and maintaining documented files and case notes which are audit ready at any time
  • Appropriately administer corrective action in conformity with IDOC regulations and Safer policy.  Corrective action should be designed to further program objectives to the extent possible.
  • Proactively participate in a variety of collective programming including the orientation of new residents.
  • Promote programs that reduce or eliminate facility’s failure to return percentage and increase facility’s employment retention percentage.
  • Participate in training that is germane to exhibiting good case management skills and operations of the Adult Transition Centers (ATCs).
  • Developing and maintaining an Individual Improvement Plan for each resident
  • Coaching the resident in everyday life and family skills
  • Maintain awareness of all facts, incidents and observations surrounding each of the assigned residents
  • Participating in staff training.

 

 

 

 

QUALIFICATIONS: 

  • Two years of case management experience
  • Good facilitation, interpersonal, and communications skills
  • Competency in Microsoft Office Suite 
  • Excellent verbal and written communication 

 

ACADEMIC BACKGROUND:

  • Bachelor’s degree
