The Re-entry Counselor is responsible for assisting participants in managing their lives to achieve successful outcomes; staying crime-free, retaining employment; and positively connecting with the community for a 6-12 month period.  Ability to speak, read and write in English and Spanish, required.

 

2 Full-Time Positions Available

 

JOB DESCRIPTION:

  • Recruit clients who meet program eligibility requirements from the surrounding geographic communities, churches, community organizations, ATCs, Safer Intake Services, Probation and Parole Departments, etc. A case load of 40 active client/ participants must be maintained and contact needs to be made on an on-going basis.
  • Conduct comprehensive client assessments and establish individualized case plans reflective of the identified needs and deliver appropriate services to recruited clients as well as those referred by faith-based and/or community-based partners.
  • Based on assessment results, provide clients with referrals that will support successful reentry into the community for assistance in areas such as housing, substance abuse treatment, healthcare treatment, mental health treatment, and legal issues.
  • Develop new and maintain existing referral sources in the surrounding communities frequented by clients and their families including keeping open lines of communications with Parole and/or Probation officials.
  • Maintain complete and accurate records of client contact in Axiom (Safer’s database) and provide regular reports on client progress to supervising manager.
  • Routine assignments and special projects are completed as required.

 

QUALIFICATIONS:

  • Three-years’ successful experience and demonstrating success in working with people with criminal records including ability to conduct staffings with appropriate partners.
  • Ability to work well under pressure in a diverse, fast-paced work environment; and, to work both independently and as a team in order to meet established deadlines. Must be flexible, self-reliant, resourceful and outcome driven.
  • Excellent oral/written/interpersonal communication skills required. Strong computer skills and familiarity with databases needed including the ability to collect/compile/prepare timely/accurate reports consisting of statistical data for both internal and external constituencies. Ability to develop marketing materials that successfully promotes the program.
  • Ability to speak, read, and write in Spanish and English is required.

 

ACADEMIC BACKGROUND:

  • Bachelor’s degree in a related discipline, preferred and 3 years’ relevant experience
