What You Will Do

Responsibilities:
- Assesses, plans and coordinates referrals, utilizing knowledge of criteria for reimbursement and qualifying criteria for facility admissions.
- Facilitiates smooth transition into or out of appropriate facility. Coordinates new referrals. Understands services/programs and regulatory/accreditation requirements to support transition of care.
- Participates in obtaining appropriate insurance authorizaiton for service, including insurance requiring pre-admission clearance and/or case management.
- Educates professional staff members, physicians and staff related to facility requirements, program requirements and range of services available.
What You Will Need
Licenses/Certifications/Registration:
- Required Credential(s):
- Licensed Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date.
- Bachelor's Level Degree
- Bachelor's degree preferred
- Three years of experience in health care marketing management required, preferably in hospice care operations.
