SUMMARY



The Case Manager will assess the psychosocial functioning of the patient including an initial comprehensive history of substance use/abuse and treatment for psychiatric conditions. The Case Manager will determine appropriate level of care pursuant to ICD 10 diagnosis and ASAM placement criteria. The Case Manager will formulate an individualized treatment plan including a discharge plan for the patient which the interdisciplinary staff will review and Attending Physician/Psychiatrist will oversee. The Case Manager will demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his/her assigned unit. The Case Manager must demonstrate knowledge of the principles of human growth and development over the life span, possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her needs and provide care as it relates to those needs that is reflected in the units/areas/department's policies and procedures. The Case Manager may also be requested to provide in-service to other staff and also supervise students.





ESSENTIAL DUTIES AND RESPONSIBILITIES:

Communicates information to the Multidisciplinary team which provides direction for treatment. This includes but not limited to: Completes psychosocial assessment using ASAM placement criteria on each patient for the purposes of admitting to the appropriate level of care and developing an individualized treatment plan. Shows proficiency in understanding adult, and/or geriatric patient. Shows proficiency in understanding adult and/or geriatric DSM V diagnoses as evidenced by the psychological assessment. Evaluates the patient for disposition upon discharge.
  • Collaborates with the Multidisciplinary team to develop a comprehensive treatment plan. This includes: Establishes goals and objectives for treatment based on assessment data contained in the Psychosocial Assessment. Conveys and documents in the EMR which will determine substance abuse treatment interventions reflecting an understanding of adult/geriatric DSM V diagnoses. Utilizes the continuum of care to meet the patient's discharge planning needs. Discharge planning reflects an understanding of special needs for aftercare (PHP, IOP, OP), Residential placement and connection with community resources.
  • Provides direct care through Group Therapy and Individual Therapy. This includes: Conducts group therapy focusing on issues unique to the adult/geriatric patient, the specific age group and the diagnoses represented in the group. Utilizes group interventions and techniques which maximize the patient group experience. Conducts 1:1 sessions which are focused on carrying out the goals of the individualized treatment plan. Conducts family session(s) as needed to provide support, education and direction for the patient and family. Provides the patient and family with information needed about substance abuse, medication compliance and support available to them after discharge.
  • Demonstrates an understanding of teamwork and contribution in order to provide comprehensive case management and programming. This includes: Consults with Medical, Nursing, Case Management and Other Consultants on clinical issues surrounding patient care. Works collaboratively with other disciplines to achieve treatment planning goals. Develops resources to assist the team in patient care. Participates in program development to enhance the patient experience and meet patient needs. Works collaboratively with other departments and/or outside agencies to insure quality of Care and access to treatment. Floats to areas/programs of high census and/or acuity as requested for supplemental staffing.
  • Participates in growth and development activities which enhance performance of duties. This includes: Attends In-Service trainings and workshops for professional growth. Demonstrates use of new and expanded skills. Shares information from workshops with treatment team.
  • Providing direct care to the adult substance abuse population. This includes: Demonstrate knowledge of evidence based practices for best outcomes in patient care. Utilize Motivational Interviewing, Cognitive Behavioral Therapy and Rational Emotive Therapy to enhance positive patient outcomes. Introduction to the 12 Step Recovery process. Facilitate didactic lectures related to substance abuse issues. Update the Plan of care in the EMR daily including progress in treatment and discharge/aftercare planning including all appropriate referrals to out-patient Providers. Facilitate family sessions and family education groups for the duration of patient's treatment. Conduct individual therapy sessions with patient for the duration of treatment.
  • Demonstrates active contribution as a team member in order to improve patient outcomes. This includes: Conduct weekly staffing with the interdisciplinary team to review patient progress in Treatment and review individualized treatment plan.
  • Professional Development. Including: Maintain all licenses and professional certifications fulfilling all educational requirements. Comply with ICB Code of Ethics and all Presence Health Ethics Policies





QUALIFICATIONS

Education and/or Experience



Master's Degree in Counseling/Social Work preferred. Experience in group and individual therapies strongly preferred. One year post-graduate experience desirable, one year of field work placement experience in a substance abuse treatment program desirable.

Certificates, Licenses, Registrations:



Certified Alcohol and Drug Counselor (CADC) required, LCPC or LCSW preferred. BA, BS required, Master's degree preferred
