Clinical Pharmacist - Pharmacy Services General 001 - Full-Time, Evening - AMITA Health - SAMC - Hoffman Estates,IL

Shift: 1:00pm - 11:30pm, 4 - 10 hour shifts each week

***Will support the Emergency Department

What You Will Do

As an Associate with AMITA Health, you will have the opportunity to
Coordinates medication use, related education, and research to provide pharmaceutical care to patients.
You will:
  • Manages drug therapy protocols as a part of an interdisciplinary team.
  • Reviews, interprets and evaluates medication orders to identify, prevent and resolve potential or actual drug-related problems.
  • Provides drug information and education to medical staff and patients.
  • Documents interventions and their outcomes for use in drug therapy quality improvement efforts.

What You Will Need

Licenses/Certifications/Registration:
  • Required Credential(s):
    • Pharmacist credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date. In good standing.

Education:
  • Bachelor's Level Degree, Bachelor's degree from an AACP accredited college required
  • PharmD preferred

Work Experience:
  • Two or more years of clinical experience required(ER or Critical Care highly preferred)
  • Minimum of PGY1, preferred PGY2 in either emergency medicine or critical care
