Advanced Practice Nurse, Inpatient Chemotherapy Service
Position Summary:
In this role, the APN works with multidisciplinary team to provide comprehensive health care to patients with cancer. Primary focus is on patients requiring an in-patient admission for the administration of chemotherapy with secondary focus of managing a small group of primary out-patients in the ambulatory arena. In addition, APN acts as liaison between the in-patient and out-patient program for oncology patients admitted to the hospital for treatment.
Essential Job Functions:
1. Acts as liaison between the in-patient and out-patient program for oncology patients admitted to the hospital for treatment.
- Participates in morning rounds sending a summary update to all oncology team members.
- Performs independent assessment of patients admitted for chemotherapy administration.
- Communicates with oncology primary team members regarding questions, concerns or changes in patient status during the in-patient stay.
- Coordinates patient and family education related to chemotherapy being administered with inpatient staff.
- Coordinates patient's discharge requirements with case management and primary team including medications, imaging requirements and follow-up appointments.
- Documents according to CMH policies: recommendations, education, plan of care.
- Performs procedures required for oncology patients during their in-patient stay as requested by the attending physician.
- Completes discharge summary for patients managed by self during the in-patient admission for chemotherapy.
2. Manages a limited number of primary oncology patients in the ambulatory setting.
- Completes comprehensive health care assessments
- Manages treatment plan for these patients including chart review preparing requisitions for tests, labs, blood bank and coordinating appointments for treatment including preparation of history/physical and treatment orders and follow up care with primary physician in the Ambulatory Center.
- Provides patient/family education regarding disease, plan of care, treatment, side effects, and requirement for follow up.
- Documents according to CMH policies: recommendations, education, plan of care.
3. Coordinates with social worker to provide support to patient and families in dealing with psychosocial issues related to their illness and therapy.
4. Coordinates outreach and teaching to inpatient and out-patient staff regarding therapy and research related to oncology.
5. Facilitates completion of research studies on selected patients as assigned.
6. Develops, reviews and revises educational tools for patients, families and staff as required.
7. Uses good communication skills; speaks and writes clearly and effectively.
8. Demonstrates strong interpersonal and teamwork skills and has ability to work with diverse constituencies throughout the medical center
9. Possesses and demonstrates outstanding leadership skills in order to be able to mentor and lead staff and mobilize and motivate disparate groups around quality improvement.
10. Demonstrates the ability to work proactively and collaboratively with people at all levels in the organization
FTE status: FT-80
Requirements
- Illinois licensure as APN required.
- At least 2 years prior NP work experience preferred.
- New Graduate NP with 3-5 years RN experience in oncology considered
