Advanced Practice Nurse, Hematology

This position provides APN support to the Hematology consultation services. In this role, the APN works with the attending physician and fellow to complete hematology consultations in a timely manner. In addition, this APN provides comprehensive health care services to all In-patients who have been diagnosed with thrombosis, require prophylaxis or treatment with anticoagulation therapy, and/or are receiving LMWH, Warfarin, Heparin, or other anticoagulants; and manages those hematology patients receiving anticoagulation therapy on an out-patient basis. Works in collaboration with the Medical Director of the Anticoagulation Team to complete initial and follow-up consultations including clinical status assessment, development of the plan of care for anticoagulation therapy, communication & follow up with the referring service to facilitate coordinated evaluation and management of anticoagulation therapy in relationship to the patients primary disease and plan of treatment.

Job Duties Specific to Hematology Consult and Anticoagulation Service APN

  1. Works with the hematology consult service to complete consultations in a timely manner.
  2. Maintains and closes EMR encounters and sends follow up letter to referring physician according to Ambulatory Service Standards.
  3. Screens and monitors appropriate labs and response to therapy of all inpatients and hematology out-patients being treated on anticoagulation therapy (e.g. LMWH, Warfarin, Subcut Heparin) and acts as resource for medical and nursing staff in the comprehensive treatment of these patients.
    • Orders laboratory studies, interprets lab results and modifies anticoagulation plan of care in the context of the anticoagulation nurse practitioner guidelines.
    • Documents according to CMH policies: recommendations, education, plan of care
    • Manages and coordinates care for patients assessed in the hematology thrombophelia clinic in conjunction with RN.
  4. Collaborates with pharmacists and dieticians in providing care to an anticoagulation patient.
  5. Provides patient and family education regarding diagnosis, anticoagulation therapy, side effects and plan of care
  6. Coordinates discharge of In-patients on anticoagulation therapy in collaboration with primary medical team and nursing staff.

o Identifies designated provider responsible for following levels and dose adjustments.

o In collaboration with patient and family identifies appropriate lab drawing facility.

o Identifies appropriate patients for at home INR monitoring. Facilitate process of obtaining home monitoring device.

o Hands off to the service who will manage the patient's anticoagulation therapy on an ongoing out-patient basis.

  1. In coordination with social worker supports patient and families in dealing with psychosocial issues related to long term anticoagulation therapy and chronic illness.
  2. Coordinates outreach and teaching to inpatient units regarding anticoagulation therapy, obtaining lab work, research.
  3. Develops, reviews and revises educational tools for patients, families and staff as required.
  4. Participates in updating policies and procedures with regards to anticoagulation therapy within the CMH.
  5. Takes leading role in identifying needs for and collaborating with other members of the anticoagulation team to continuously improve processes related to this program and meet accreditation standards.
  6. Facilitates participation in clinical research and Anticoagulation Program outcome measures.
  7. Uses good communication skills; speaks and writes clearly and effectively.
  8. Demonstrates strong interpersonal and teamwork skills and has ability to work with diverse constituencies throughout the medical center
  9. Possesses and demonstrates outstanding leadership skills in order to be able to mentor and lead staff and mobilize and motivate disparate groups around quality improvement.
  10. Demonstrates the ability to work proactively and collaboratively with people at all levels in the organization

FTE status: FT-80

Requirements

  • Master's degree from an accredited school of nursing appropriate for certification as a Nurse Practitioner is required.
  • New grad NP with pediatric or family practice focus considered; interest in developing a field of expertise;
  • 5+ years of previous Hem/Onc nursing experience required, peds experience preferred.
  • Licensure in the State of Illinois as an APN or eligible for certification is required.
  • Basic Life Support certification and Pediatric Advanced Life Support certification required.

HOURS: Monday-Friday, 8a-5:30p (Flexible Hours)

REQUIRED: 5+ years of Hem/Onc RN experience required for this position; New grad NPs may beconsidered with this background.
