Car Required Driver s license, automobile and 100/300/100 auto liability insurance required.
Position Hours Full-time; Monday - Friday, 8;30am - 5:00 pm.

Description of Duties:
Teach and assess independent living skills to youth with a diagnosis of mental illness. Attend clinical staffings and other clinical meetings as necessary. Assist youth move from a group home to an apartment and transition to an adult placement when the youth leaves the program. This person will also be required to provide documentation of services and achieve productivity goals with attention to the clear demonstration of medical necessity.
Qualifications and Experience Required:
This person should have good written and verbal communication skills and strong basic computer skills, which include the ability to document services in a clear and effective manner. Experience in the mental health field is strongly preferred.
Requirements, Responsibilities and/or Duties Unique to the Position:
Driver?s license, automobile and 100/300/100 auto liability insurance required.
