About the Role

A Corporate Account Manager (CAM) is responsible for placing participants into quality permanent jobs by identifying, developing, and managing a participant pipeline while collaborating with established Cara employment partners.

Reports To

Manager of Placement Services

Outcomes

The Placement Team is responsible for securing job opportunities for participants, and providing concierge level service to our employment partners. Corporate Account Managers are required to maintain key performance indicator metrics including monthly job orders, candidate recommendations, and site visits with the ultimate goal of placements.



The Responsibilities of the CAM team

* Play a vital role on the Placement Team in securing quality permanent job placement for Cara enterprise participants

* Partner with Senior Corporate Account Manager to drive results towards placement goals

* Manage a portfolio of employer accounts across multiple sectors, to build strong partnerships and create scalable talent pipelines

* Coach and guide participants to review job requirements and other employer expectations

* Advise participants regarding application process, interview preparation and offer/acceptance phase of job search

General Summary:

Reporting to the Clinical Director, the Substance Use Counselor will be a critical component to the successful launching of substance use and mental health outpatient treatment services within Safer's community sites, starting with the location of 808 S. Kedzie. The Substance Use Counselor will be a key member of the treatment team providing assessments, treatment planning, and counseling to clients referred for services, as well as contributing to the overall design, start-up, and implementation of services.

Essential Duties and Responsibilities:

* Conduct assessments using standardized assessment tools.

* Based on assessment results, provide appropriate referral to other service providers inside and outside of Safer.

* Develop treatment plans that are appropriate to the needs of the individual.

* Facilitate individual and group counseling sessions with groups; possibly evening groups.

* Provide group education.

* Complete clinical documentation and progress notes in a timely manner consistent with contract requirements, agency, state, and federal guidelines.

* Complete individualized treatment plans for each client within guidelines set.

* Complete treatment plan reviews for each client at least monthly.

* Administer, interpret and document urine analysis drug testing.

* Adhere to agency policy and procedures.

* Maintain high standards of professional and ethical conduct.

* Complete training and/or quality assurance activities within given time frames.

* Establish and maintain a good working relationship with community stakeholders/referral sources.

* Complete discharge, discharge assessments and discharge summaries for each client.

* May serve as back-up for treatment staff inside the Adult Transitional Centers.

Required Knowledge, Skills and Abilities:

* Knowledge of human behavior and performance; individual differences in ability, personality, and interests, learning and motivation; and the assessment and treatment of behavioral and affective disorders

* Knowledge of principles, methods, and procedures for treatment and rehabilitation of substance abuse

* Knowledge of group behavior and dynamics and societal trends and influences

* Computer/Technology Literate: Knows how to leverage the internet and computer applications to obtain information

* Experience using electronic health record systems to document services and progress notes. Special consideration will be given to candidates with experience using CareLogic

SUMMARY

The Staff Nurse I - Registered Nurse is a Registered Professional Nurse (RN) who practices within the scope of the Illinois Nurse Practice Act to ensure the delivery of comprehensive, high quality, individualized nursing care, based upon knowledge and experience, the nursing process and leadership skills. PREFERRED EXPERIENCE:

1. Minimum of 2 years Critical Care experience, (of those 2 years, at least 1 year specifically with the Cardiovascular Surgery Patient Population)
a. Coronary Artery Bypass Graft
b. Valve Surgery
c. Open Aneurysm Repair/Graft
d. Thoracic Surgery
e. Intra-Aortic Balloon Pump
f. *Impella, ECMO, and TAVR experience preferred but not required*
2. Knowledge of post-operative hemodynamic monitoring required
3. Must speak to possible post-operative complications

SUMMARY

The Staff Nurse I - Registered Nurse is a Registered Professional Nurse (RN) who practices within the scope of the Illinois Nurse Practice Act to ensure the delivery of comprehensive, high quality, individualized nursing care, based upon knowledge and experience, the nursing process and leadership skills. The RN incorporates the Carative Factors of human care and performs established procedures in accordance with policies, procedures and standards of care of Presence health and the nursing division. The RN coordinates family involvement in the assessment, treatment and continuing care of the patient, and delegates tasks to competent and appropriate caregivers.

ESSENTIAL DUTIES AND RESPONSIBILITIES

* Assesses each patient's biophysical, psychosocial, environment, spiritual, cultural, self-care, educational and discharge planning based on age and development status.
* Develops and prioritizes an outcome directed plan of care to assigned patients and changes as necessary.
* Performs clinical and technical aspects of care in accordance with established policies, procedures, standards of practice and within scope of the State Nurse Practice. Documents care per established standards.
* Evaluates the effectiveness of nursing interventions. Modifies the plan of care focusing on age and development status and patient outcomes.
* Assess patient and family educational needs and ability to participate and learn to achieve patient plan of care through collaboration with other team members.

PREFERRED EXPERIENCE:

1. Minimum of 2 years Critical Care experience, (of those 2 years, at least 1 year specifically with the Cardiovascular Surgery Patient Population)
a. Coronary Artery Bypass Graft
b. Valve Surgery
c. Open Aneurysm Repair/Graft
d. Thoracic Surgery
e. Intra-Aortic Balloon Pump
f. *Impella, ECMO, and TAVR experience preferred but not required*
2. Knowledge of post-operative hemodynamic monitoring required
3. Must speak to possible post-operative complications

SUMMARY

Under the direction of the Pharmacy Director, Manager, Supervisor and/or Pharmacists, monitors and maintains drug and supply inventory for all areas in Pharmacy including automated packaging and dispensing equipment. Ensures purchases are in compliance with 340b laws, regulations and policies for all eligible locations.

ESSENTIAL DUTIES AND RESPONSIBILITIES

According to established policies and procedures, prepares and submits orders to approved vendors to purchase medications, IV fluids, and pharmaceutical-related supplies including controlled substances.

Monitors incoming stock orders. Verifies orders are complete and identifies discrepancies from original order. Receives, unpacks, and stocks product in appropriate areas. Follows up with vendors on invoicing and order discrepancies. Submits invoices to finance for payment.

Maintains and rotates inventory. Monitors expiration dates of medications, IV fluids, and supplies. Removes outdated and recalled stock from pharmacy inventory. Returns unused medications to vendor and submits for credits when necessary.

Works with management to establish and maintain adequate inventory levels. Informs management of back orders, out of stock items, and recalled items and arranges for substitutes when needed. Notifies management when substitutions are unavailable.

Follows-up with outside vendors on appropriate purchasing needs including alternate sources for short supply items, product availability, and shipment information. Obtains pricing information for price comparison between vendors.
